The new rule, which takes effect Jan. 1, 2017, requires employers with more than 20 employees in certain high risk industries to electronically submit injury and illness data that they are already required to record on their onsite OSHA Injury and Illness forms.
OSHAFormz mobile is a field data collection solution that personnel incharge of safety can use the app to capture work-related injury and illness incidents directly in the field.
OSHAFormz automatically calculates the totals and populate the summary page (Form 300A). The app has the ability to generate the OSHA PDF forms and also CSV files. These files can be exported and uploaded directly to OSHA Injury Tracking Web Application (ITA).
The free version allows users to create 1 Form 300/300A and 2 Form 301s.
OSHAFormz offers auto-renewable subscriptions which allow the user to create more Form 300/300A, and Form 301 records than the free version.
The following 3 types of paid subscriptions are available:
Tier A - ($14.99/6 Months OR $24.99/Year) - Allows 2 Form 300/300A and 7 Form 301 per each Form 300/300A. Ideal for Small Businesses.
Tier B - ($29.99/6 Months OR $49.99/Year) - Allows 4 Form 300/300A and 7 Form 301 per each Form 300/300A. Ideal for Medium Businesses.
Tier C - ($69.99/6 Months OR $99.99/Year) - Allows unlimited Form 300/300A and unlimited Form 301 per each Form 300/300A. Ideal for Medium and Large Businesses.
Note these important aspects of auto-renewable subscriptions:
– Payment will be charged to iTunes Account at confirmation of purchase and your credit card will be charged through your iTunes Account.
– Subscription automatically renews unless auto-renew is turned off at least 24-hours before the end of the current period
– Account will be charged for renewal within 24-hours prior to the end of the current period, for the respective tier.
– If subscription is not renewed, new Form 300 or Form 301 records cannot be created. Existing records will continue to be available.
– Subscriptions may be managed by the user and auto-renewal may be turned off by going to the users Account Settings after purchase.
– Any unused portion of a free trial period,will be forfeited when the user purchases a subscription.
